Broken threads on LS

Has anyone come across this issue? The actual threading is coming off of the hole to attach the LS to any of the adapters. Not my unit, this was a loaner while ours is getting internal radio antenna fixed, not sure how the problem started, to my knowledge it hasn't taken a drop or been mishandled since we got it. The first time the thread popped out and I was able to get it to go back in, but yesterday seems to have been the last straw. Unable to get the threads to bite at all, otherwise I would just get one of the adapters on there and leave it.

Thoughts? Suggestions?

I imagine a lot of hard use on the rentals. Bring it to an auto parts store and get a helicoil to replace it.
